There are different types of evaluations. Each evaluation has its own questionnaire according to its type.

Questionnaires are made on this page.



Create new Questionnaires
To create new Questionnaires, click on the Image Added. note that:

"Title" is the name given to the questionnaire.

"Minimum score", is the minimum grade that can be given to each question.

"Maximum score", is the maximum grade that can be given to each question.

To assign a question to the current questionnaire, click on the table at the bottom of the page.

In the first column, by pressing Image Added, you will see a list of questions defined in the Question Bank. Click on the desired question and finally click on Image Added.

In the second column, "Answer" is not selectable and is filled in according to the selected question.

In the fourth column, by clicking on the arrow, you will see a list of empowerments defined on the Empowerment page.

Note that in this list, competencies are bold and  are not selectable, and the bottom lines of competency are empowerment related to that competency.

"Order" indicates the position of the question in the current questionnaire.

To create a new line, click on the last blank line, and to delete a line, first select it and then click on Image Added.

At the end, to save above items, click on the Image Added.

Edit a Questionnaires

To modify a Questionnaires, select it from the list, click on the Image Added button, make the desired changes, then click on the Image Added button.

Delete a  Questionnaires

To delete a Questionnaires, select it from the list, click on the Image Added button, If you are sure of deleting the selected item click on the Image Added button.

Note: if the selected Questionnaires is used in anywhere, you will not be able to delete it. You can make it inactive by deselecting the "Active" checkbox.

After defining a questionnaire, with the explanations mentioned above, the preview of a complete questionnaire can be viewed by clicking on Image Added.

In the opened page you will see a defined questionnaire. On this page, it is possible to view the designed questionnaire with different categories.

For example, if you want to categorize a questionnaire by question value, it displays all questions with the same score in one category.
